Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Our commitment to your privacy

Zeva Care, PLLC is required by law to protect the privacy of your protected health information (PHI), to provide you with this Notice of our legal duties and privacy practices, and to follow the terms of the Notice currently in effect. PHI is information that identifies you and relates to your past, present, or future physical or mental health, care, or payment for care.

How we may use and disclose your information

We may use and disclose your PHI without your written authorization for the following purposes:

  • Treatment — to provide, coordinate, and manage your psychiatric care, including conducting telehealth visits, prescribing and managing medication, and consulting with other providers involved in your care.
  • Payment — to bill and obtain payment for your care, including verifying insurance coverage, determining eligibility, and processing the payment method you keep on file.
  • Health care operations — to support our practice, such as quality review, staff evaluation, scheduling, and administrative functions.
  • Appointment reminders and care communications — to send you confirmations, reminders, and your telehealth visit link.

We may also use or disclose your PHI when required or permitted by law — for example, for public health activities; to report suspected abuse or neglect; for health oversight, judicial, or law-enforcement purposes; to coroners or medical examiners; for serious threats to health or safety; for workers’ compensation; and for specialized government functions.

Uses that require your authorization

Most uses and disclosures of psychotherapy notes, uses or disclosures for marketing, and any sale of your PHI require your written authorization. Other uses not described in this Notice will be made only with your written authorization, which you may revoke at any time in writing, except to the extent we have already relied on it.

Special protections for mental health and substance use records

Mental-health records receive additional protection under the Pennsylvania Mental Health Procedures Act, and records relating to substance use disorder treatment may be further protected under federal law (42 CFR Part 2). Where these laws are more protective than HIPAA, we follow the stricter standard.

Your rights regarding your health information

  • Access and copies — you may inspect and request a copy of your medical and billing records.
  • Amendment — you may request that we correct information you believe is inaccurate or incomplete.
  • Accounting of disclosures — you may request a list of certain disclosures we have made of your PHI.
  • Request restrictions — you may ask us to limit how we use or disclose your PHI. If you pay in full out of pocket for a service, you may request that we not disclose that information to your health plan.
  • Confidential communications — you may ask us to contact you in a specific way or at a specific location.
  • Paper copy — you may request a paper copy of this Notice at any time.
  • Breach notification — you have the right to be notified if a breach of your unsecured PHI occurs.

Telehealth

Zeva Care delivers care by telehealth. Your video visits take place over an encrypted, HIPAA-compliant platform. We apply the same privacy protections to information created during a telehealth visit as we do to any other PHI.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with us using the contact information below, or with the U.S. Department of Health and Human Services, Office for Civil Rights, at hhs.gov/ocr. We will not retaliate against you for filing a complaint.

Changes to this Notice

We reserve the right to change this Notice and to make the revised Notice effective for all PHI we maintain. The current Notice will always be posted on this page with its effective date.
